How can I format a report like this?

I've been playing around with it and either it isn't possible or I just don't know how to do it. Some guidance would be greatly appreciated.

I need to somehow come up with a report layout like this [simulated]:

Specifically I need to somehow create multiple columns of data... three columns across with the header information over to the left displayed once for each row.

The closest I can get is below which gives me multiple columns of data but unfortunately has the header information in each column:

I would like to display that header information only once per row while still having multiple columns of data. Is this possible? Could somebody help me with an example if so?

And THIS is one of the many reasons we decided to go with you guys... because your customer service is top notch! Thank you so much for taking the time to make an example. We are very grateful!

There is one problem with the example though. If you'll look at the screenshots below you'll notice that it hides the 1st, 5th, 9th, etc... entries. Their should be 12 entries displayed (so 4 rows of 3 totaling 12) but since the example is essentially "hiding" the 1st, 5th, 9th, etc... entries there are only 9 out of the 12 entries displayed. Is there a way to do this where we get the 4 rows of 3 showing all 12 entries instead of 3 rows of 3 showing only 9 of the 12?

And you can see here that there are three entries that don't end up getting output in that example.

Is there a way to tweak that example to where the look/format is virtually the same but without missing/hidden entries? Thanks again. You guys rock!

You know what... never mind that. I figured out a way to do it. I moved the data fields over to where they don't start behind the headers but they start to the right of the headers. I then made it 3 columns instead of 4 and made sure to not hide that first column. It worked out beautifully. The key though was the conditionals on the headers... hiding them on every column except for the 1st. I didn't know how to do that 'till you gave me the example so thanks a ton for the example. That got me headed in the right direction and definitely opened up my eyes to the conditions abilities that I had not explored before.

I uploaded the updated report just in case anybody else runs across the same or similar scenario.
