Starting documentation from Reports

Hello everyone! I work as a systems analyst at a hospital in Brazil. My department is responsible for creating customized reports to meet the needs of the healthcare sector, providing the necessary information.
One of our main challenges is the lack of documentation for every report we deliver. For example, maintaining a report created by a former employee is really difficult.
Does anyone have experience they can share regarding documenting projects created with Stimulsoft Reports? What would be a good structure for such documentation to cover both the SQL query and the data layout (which follows the Stimulsoft standard)?
It's relatively easy to find a template for SQL queries that fit the report, but I’m finding it quite challenging to create something that accommodates the features provided by Stimulsoft software.
