Complex Printout

Hi,

I have a requirement here to convert an complex manual form to electronic.

Now I need to print this form out. Its a form where each alphabet got a box to it. Then there is a bunch of Tick boxes. I hope you understand what I mean.

Is there any technique how should I design this printout ?
